In order to apply for U nonimmigrant status, a qualifying alien needs to submit the following documents:
Form I-918, Petition for U Nonimmigrant Status
Form I-918, Supplement B, U Nonimmigrant Status Certification
This form is to be filled by law enforcement officials in order to confirm that the alien was or will likely be helpful in criminal investigations or prosecutions
A personal statement describing the criminal activity of which you were a victim
Evidence establishing each eligibility requirement
In order to petition for derivative U status for a qualifying family member, an alien must also file a Form I-918, Supplement A, Petition for Immediate Family Member of U-1 Recipient. You may download Form I-918, Supplement A here. Form I-918, Supplement A should be submitted at the same time as the principal’s application.
There is no filing fee for those petitioning for U nonimmigrant status. Documents must be submitted to the Vermont Service Center by sending them to the following address:
